We now have three versions of surveys to evaluate your Prevention Plus Wellness program.
First, the original pretest and posttest surveys come in both online and paper copy formats. The updated online surveys now allow participating youth to enter a personal code to match online pretest and posttest surveys and a school/location code to assess program effects by school or location.
Second, we have an online pretest-posttest survey you can use to enter your paper copy pretest and posttest survey data into for easy analysis.
Third, we have a behavioral Youth Health Survey for organizations that wish to collect pre-program and post-program follow up measures of health behaviors to assess behavioral change resulting from your PPW program.
